To all Chabot families,

Welcome and Welcome back! It’s time for the Chabot School Directory annual update. The PTA Directory committee invites you to submit your entry. Many parents describe the Directory as “invaluable.” Don’t miss your chance to be included. Please complete form even if you have no changes from last year. It makes it much easier for us to compile the directory.

The PTA does not share this information with anyone outside the school or sell this information to anyone.

Silence is consent! If you do not want your child’s information included in the Directory you must notify us in writing. If you respond in writing please place your information in one of the Directory boxes.

Please complete the form and deposit it in one of the Directory Boxes. They are located in the school office and at Adventure Time. The forms will be distrubuted to your children.

The sooner we get everyone’s information the sooner the Directory comes out!
